The Anesthesia Question Cosmetic Surgery Patients Forget to Ask

Patients comparing plastic surgeons ask about technique, scars, recovery time and price. Far fewer ask who will manage their airway during the hours they are asleep.

That question separates practices more than the others do.

Two variables a quote hides

Two things sit inside every cosmetic surgery estimate and neither appears as a line item: where the operation happens, and who administers the anesthesia.

Surgery can take place in a hospital operating room, in an accredited ambulatory surgical facility, or in an office suite held to a lower bar. Accrediting bodies set standards for equipment, emergency protocols, staffing and recovery monitoring. A facility carrying accreditation has opened itself to inspection against those standards. A facility without it has not.

Anesthesia splits the same way. A physician anesthesiologist is a medical doctor who completed a residency in anesthesiology. Certified registered nurse anesthetists and anesthesiologist assistants come through different training paths, under supervision rules that vary by state.

Leo Lapuerta, MD, FACS, a triple board-certified plastic surgeon serving the Houston metro, owns and operates a QuadA accredited surgical facility in Pearland and staffs every case with a physician anesthesiologist. His practice uses no nurse anesthetists and no anesthesia assistants.

Both choices raise what a case costs. Both change who stands at the head of the table.

Why the low quote is low

A facelift quote landing well under the Houston market points to something other than a faster surgeon. It reflects a shorter operation, a facility at a lower level of accreditation, or a different anesthesia arrangement. Patients who ask which of the three explains the discount learn more than patients who compare totals.

Privileges as a second signal

Board certification tells you a surgeon passed the examinations of a certifying board. Hospital privileges tell you a credentialing committee reviewed that surgeon’s training, case log and outcomes, then agreed to let them operate inside the institution. The review repeats on a cycle.

Dr. Lapuerta holds privileges across the Memorial Hermann network, at HCA facilities including The Woman’s Hospital of Texas, and at St. Luke’s Houston Medical Center. He serves as Chairman of Plastic Surgery at St. Joseph Hospital and holds faculty appointments at UT Houston, UT Medical Branch in Galveston, and the Houston Methodist plastic and reconstructive surgery fellowship. He is a Fellow of the American College of Surgeons, a member of the American Society of Plastic Surgeons and the Texas Society of Plastic Surgeons, and a past President of the Houston Society of Plastic Surgeons.

Four questions before you book

Asked at the consultation, four questions surface most of what a price sheet leaves out.

  • Where will my operation take place, and which body accredits that facility?
  • Who administers my anesthesia, and what is their training?
  • Which hospitals have granted you privileges?
  • How many of this specific operation have you performed?

A practice that answers all four without hesitation has answered them before. Dr. Lapuerta has performed more than 30,000 surgical and non-surgical procedures across thirty years in Houston and publishes more than 3,000 before and after photographs for patients to review.

Patients weighing a facelift, neck lift, tummy tuck or mommy makeover in the Houston metro can review procedure detail and photographs at lookyounger.net. The practice sees patients in English and Spanish, and the clinical staff is bilingual.
